"Don't Learn Safety By Accident" - A Wise Person (probably)

It is important to emphasize to your team the value of a proactive, healthy, positive safety culture. Near misses and minor injuries (see above section) may be frustrating, but effective reporting of them is a great indicator of a positive safety culture. 

Don't think that no reported incidents means our programs are 100% safe. This is an indicator of complacency, and it means you are not aware of the warning signs of a potentially more serious incident in the future.
