The below document describes our Gating Factors. We use these factors to structure conversations around program start up and, as a basis to engage the Crisis Management Team (CMT) as the situation evolves throughout operation.
Gating factors do not equate to decisions, they start conversations. We have developed "red lines" to describe situations which would require actions such as operational pauses, or even closures.
The below flowchart shows the process of monitoring the gating factors and how, when issues are detected, they are escalated.
