The business process requires an applicant that requests service but is renting the property to get consent by their landlord. This consent is submitted through the Tenant Authorization Form (TAF) and besides that consent also informs the applicant that a Lien Offset Payment (LOP) of currently $190 is to be submitted prior to the service being started in their name.
Step 1: Navigate to the following URL: https://www.fwcustomer.org/fairfax. Customer will enter their 1) User Name and 2) Password, then click Login.
Step 2: For TAF, users click on Start Service in the left navigation bar.
Step 3: Users will go through 3 steps for submitting TAF:

Users must tick the box to indicate that they have read the terms and conditions then click Next button in order to navigate to the next step
2. Service Address;
Users must choose if they are an owner, landlord or tenant. to submit a TAF users select Landlord. then click Tenant Authorization Form button.

User will be navigated to step 3 as seen in the below screenshot, user first provide the address by filling out the following fields:
House Number
Unit
Street Name
City
Zip Code
After entering the address, users have to enter the date they would like the service to start on
Tenant Information: provide the information requested below for each tenant as seen below:
then user fill out Property Owner or Authorized Agent & Mailing Address information as seen in the below screenshot:
lastly, user fill out the Acknowledgement & Signature in order to submit the TAF, then click submit button
users are able to download a PDF version of TAF to fill out offline if needed.

Step 4: Upon submitting the TAF, a conformation popup is displayed to the user
Step 5: Also, an email is sent to the user once submitting a TAF to acknowledge that the user has submitted a TAF
Step 6: the submitted TAF will be sent to TAF team to review and process
Step 1: Non-logged in users are able to submit TAF through homepage by clicking on TAF button
