Resources - Injury Report

This resource is for a staff member to report an injury to themselves. It is not used to report student injuries. This resource needs occasional updating. Check with the district office to verify it is the most recent version. This page was last updated for the 2025-26 school year.

All workplace incidents involving staff should be reported by calling 1-844-322-4668. You will need to identify yourself as a Baldwin-Woodville employee. The vendor will document the incident details and complete all the required paperwork. Questions about this policy can be directed to Michelle Sell.
