Resources - Hotspot Form

Hotspots are mobile WiFi routers we have leased through T-Mobile.  The hotspots are intended to lessen the digital divide for students who are in need. 

An early step in allowing a student to check out the hotspot is having their parent/guardian fill out this form. 

Administrators need to authorize the hotspot check out so it may be a good idea to have a conversation with them about the student's needs before sending this form home.

It is also a good idea to send a message to helpdesk@bwsd.k12.wi.us asking if a hotspot is available and requesting the serial number of a hotspot that should be put on the form.  

After your administrator has authorized a family to receive a hotspot the completed form on this page should be sent to someone in the IT department.   They will direct you how to get the hotspot.
