GRADUATE TRANSCRIPT REQUEST

If you are a graduate of Luxemburg Casco High School or have attended in the past and need a copy of your transcript-please contact the Counseling Department Administrative Assistant-Kelly Addison at 845-2336 x460 or kaddison@luxcasco.k12.wi.us.

Please provide the following:

• Year you graduated or years of attendance

• Maiden name

• Indicate if you need an official or unofficial transcript (official transcripts must be sent from LCHS directly to another institution)

• Where you would like your transcript sent
