Entering a New Course Petition

This Qualtrics form is maintained by the Eccles School Salesforce Team, not UAS. Changes should be requested through the Case process in Salesforce.

1. Go to Qualtrics submission form (Undergrad.Eccles.Utah.Edu > Admissions > Transfer Students > Transfer Equivalency Guide).

2. Enter in required information, including a syllabus. Domestic or International petition choice will be options in the Qualtrics form and will have separate websites when searching for existing equivalencies.

Once all information is entered, the petition has been submitted through this form and will populate in the "Course Petitions" tab section in SF. The petition status will say "Student Submitted."
