Open Enrollment Individual Registration Form

The registration form collects information about the individual registering and sends the information to Salesforce. Please see the information below to determine the destination object of specific data.

To add or remove a company from the organization list, navigate to the organization in Salesforce and (un)check the "Exec Ed Picklist Display Company" box.
