Dear Sir,
Please provide the information with details and copy of documents attached on each of the issues listed below.
1. David Thomas Report
a) A copy of the David Thomas Report
b) A copy of the detailed terms of reference given to Sri David for writing this report
c) The payment given to him on account of (I) Honorarium, (II) boarding and lodging (III) salary or stipend or Fellowship paid to Sri David (IV) Payment under any other head.
d) Total days spent by him on the Report making and total money spent on Sri David during his stay.
e) In case he was made a 'scholar in residence' at that time please clarify whether he was on the institute rolls.
2. Death by fall of migrant- worker Sh. Ram Saran Rai on November 21, 2012
i. A copy of the Enquiry Report with ALL appendices, Annexures by Sh. S. R. Pandey, ADG, CPWD (retd.) appointed vide Office Order Dir/IITK/2012/00-98, November 21, 2012.
ii. Amount of compensation paid to victim’s family directly.
iii. Amount of compensation paid to victim’s family through the Labour Commissioner.
iv. If answer to (ii+iii) is less than legal compensation due to his family, please indicate the compensation amount deposited with the Labour Commissioner.
3. Death by fall of contract worker Sh. Atar Singh on October 18, 2012:
a. A copy of the one-man enquiry committee report with all appendices and annexures appointed by the institute vide Communique IITK/DC-88/LC 19th October 2012.
b. Amount of compensation paid to victim’s family directly.
c. Amount of compensation paid to victim’s family through the Labour Commissioner.
d. If answer to (b+c) is less than legal compensation due to his family, please indicate the compensation amount deposited with the Labour Commissioner.
Please send the exact cost of copying etc so that the payment can be made w/o delay.
