Displays a Priorities field on a form, along with an Objectives field.
The Priority(ies) selected determine which Objectives are available, as configured in the Priority and Objective lookup lists in Admin > Lookups.
Configuration options:

* Priority is required
* Objective is single select
* Objective is required
