MEDICATION POLICY
To promote the safety of all school children, it is important to review the medication policy of the Lewiston Public Schools. The purpose of this policy is to provide a safe procedure for the administration of medications to students in the school setting.
When possible, medications should be given to students at home. When it becomes necessary for medication to be given during school hours, the following procedure must be followed:
Medication will be brought to school by a responsible adult/person.
Medication will be properly packaged with the original pharmacy label which shall include the following information:
Student name
Ordering Physician
Date
Medication name
Medication dose
Method of administration
Quantity
We must have written orders from the doctor/provider and the parent/guardian must sign a permission form (Medication Authorization Form) requesting that the school comply with doctor’s orders.
If there has been a change in medication, dosage or time interval, a written order from your doctor/provider will be required to prevent any error or confusion.
Verbal instructions from the student or parent are not acceptable and at no time shall a student have medication in his/her possession.
Inhalers may be self-administered for respiratory problems only with a note from the child’s provider stating the child has been fully instructed in its use and is capable of using it as directed. The school nurse will develop individual health plans with consideration of age, development, understanding and reliability.
It is very important that each parent comply with this policy for the health and safety of your child. Questions may be addressed to the school nurse in your child’s school.
