Child Abuse Reporting Policy

Please see attached revised instructions and reporting template for both CAR and PSB-CY reporting.

Please use the attached form (V3) effective Friday, 27 Sep 19.

Summary of Changes for CAR:

1. Date FAP, LLE, and CWS contacted.

2. Verification CWS is not provided in Japan, Okinawa, and Korea.

3. Additional options for the type of medical treatment.

4. Revised reporting procedures/instructions for clarity.

PSB-CY Reporting:

1. When reporting PSB-CY to FAP submit a SIR in Service Now. (See your administrator to do this step.)

2. If FAP requires a written report of the incident either use the CAR form (since there is not a DoDEA PSB-CY reporting form) or download and print the PDF of the SIR from Service Now.

3. If using the CAR form to report a PSB-CY incident to FAP DO NOT submit as CAR via DoDEA reporting channels; only submit a SIR.
