Before the fog there were many Bishops to one church but after the fog there was one Bishop to many churches. This laid the foundation for a hierarchy, and when you get a hierarchy you always get a low-rarchy and the low-rarchy don't minister. They always become spectators of the hierarchy.
The hierarchy then build a pyramid, and this is not what Jesus or Paul did. Monopoly of ministry came in and you had to achieve a certain level before you could minister.
Then followed, special building, special clothes and the rest is history.
