Leaving Employment with DNR
A message from PERA for anyone retiring in the next couple of years:
General Information:
Whether you are retiring or resigning from DNR there is important information you should be aware of:
You will be paid out your annual leave balance, up to the maximum accrual. This payment will occur in the month following your departure. If you are resigning, no sick leave is paid out. If you are retiring, ¼ of the balance of your sick leave (up to your personal maximum) will be paid out. Contact DNR's payroll group (DNR_Payroll@state.co.us) if you have questions regarding this calculation.
Your benefits end on the last day of the month in which your employment ends. You may continue to carry the medical, dental and flexible spending account benefits through COBRA. This information will be sent to your home address within 30 days after your last day of employment.
You may also port or convert coverage of your life insurance by contacting Minnesota Life at (877) 828-7728.

Please complete either a Resignation Form, or Retirement Form, and give it to your supervisor. Your supervisor should then complete an EDSyS transaction to notify the HR Operations team of your resignation/retirement and send the completed Resignation/Retirement form to DNR_HR_EmployeeTransactions@state.co.us.

Manager/Supervisor Responsibilities:
Once your employee has made the decision to Resign or Retire, please have them complete the Resignation Form or Retirement Form and send it to your HR Specialist and DNR_HR_EmployeeTransactions@state.co.us
Acknowledge their Retirement/Resignation via email by stating "I accept your Retirement [or Resignation]."
Initiate the EDSys transaction to separate your employee from State employment and send the completed Resignation/Retirement Form to DNR_HR_EmployeeTransactions@state.co.us.
Supervisors and managers, if you are processing a separation please utilize the Exit Checklist.

Step 1: Submit an DNR IT Offboarding - IT Resource Deprovisioning request
This DNR IT Offboarding - IT Resource Deprovisioning form is used for:
Account deletion (NATURENET/google)
Personal/Home Drive - Deletions/Change of permissions
Phone extension deprovisioning ticket creation
Equipment deprovisioning ticket creation
If you have any questions about IT Resource deprovisioning please contact your Department Identity Access Manager (IAM)
liaison. Do not submit a ticket to OIT.
CPW Hiring Managers
Step 1: Offboard User Account/Email
Please refer to the following instructions on using IAM.

Step 2: Offboard Equipment:
When a CPW employee is off boarded they are required to return the following items to their manager:
Desktop or laptop, monitor and all peripherals, tablet, IPAD and cell phone.
Managers please request the pin(s) to unlock cell phones, tablets, IPADS and apple ID passwords if applicable.

Retiring:
When you decide to retire, please contact PERA first as they determine retirement eligibility. Please keep in mind that your retirement date for PERA and other benefits is the first of the following month of your effective date of resignation/retirement. For example:
If your effective date of resignation/retirement date with the state is March 15th, your retirement date is April 1st.
If your termination date is February 28, your retirement date is March 1.
Before making a retirement date decision the employee should contact PERA to discuss options. Once you have confirmed retirement eligibility with PERA, please notify your supervisor as soon as possible and complete the Retirement Form.
Other things to keep in mind:
Complete your KRONOS time-sheet as early as possible with any requested leave, to the best of your knowledge, and approve. Changes can be made, if necessary, to be coordinated with your supervisor.
Have your supervisor change any "requested leave" to actual leave, and approve.
You will receive your normal direct deposit on your normal pay cycle for the final pay period of your employment
DNR's payroll group will provide you an email that includes the final figures for your payout, along with a copy of your Final Six Month Salary report, as submitted to PERA, and the calculation of your final leave balances.
Please contact PERA for coordination of benefits such as PERACares. Your benefits with the State will end on the last day of the month in which you were employed.

COBRA Information:
The Consolidated Omnibus Reconciliation Act of 1985 (COBRA) allows employees and their covered dependents to continue group health coverage under certain circumstances. The State uses a COBRA third-party administrator (TPA). The TPA is responsible for sending COBRA notices and for managing COBRA enrollment, billing, payment, and cancellation. Your COBRA coverage will become effective once your first premium is received by the COBRA TPA. Coverage is retroactive once payment is received.
COBRA is a way for you to maintain the benefits you had while employed with the State. The premium costs are Total Premium (Employer + employee contribution) + a 2% administration fee.
