When a CPW employee is off boarded they are required to return the following items to their manager:

Retiring:

When you decide to retire, please contact PERA first as they determine retirement eligibility.  Please keep in mind that your retirement date for PERA and other benefits is the first of the following month of your effective date of resignation/retirement. For example:

Before making a retirement date decision the employee should contact PERA to discuss options.  Once you have confirmed retirement eligibility with PERA, please notify your supervisor as soon as possible and complete the Retirement Form.

COBRA Information:

The Consolidated Omnibus Reconciliation Act of 1985 (COBRA) allows employees and their covered dependents to continue group health coverage under certain circumstances. The State uses a COBRA third-party administrator (TPA). The TPA is responsible for sending COBRA notices and for managing COBRA enrollment, billing, payment, and cancellation. Your COBRA coverage will become effective once your first premium is received by the COBRA TPA. Coverage is retroactive once payment is received.

COBRA is a way for you to maintain the benefits you had while employed with the State. The premium costs are Total Premium (Employer + employee contribution) + a 2% administration fee.
