Sleep Medicine and Pain/Coping

Objective: to provide exposure to clinical experiences in sleep medicine as well as in the multidisciplinary medical management of chronic pain in children that includes time in pain clinic, coping clinic therapy sessions and physical therapy.

Elective description:

The main focus of this elective is to further train pediatric residents in the evaluation of and multidisciplinary approach to treatment of sleep disorders and chronic pain in children. Residents often see children in the hospital or in the general pediatrics clinic who have chronic pain that may be affected by poor or inadequate sleep; though many residents refer patients to sleep, pain or coping clinic, most have little practical knowledge about the strategies that are used to evaluate and/or treat sleep disorders or chronic pain. General pediatricians frequently encounter children with sleep or chronic pain disorders; thus, gaining expertise in this area is crucial to improving physician knowledge base and expertise. This elective will emphasize the development of three main areas in which a future general pediatrician should be skillful: obtaining a targeted sleep and/or chronic pain history, critically evaluating sleep study data and previous lab data, and utilizing a multi-disciplinary approach to treatment. The rotating resident will gain expertise in these skills through autonomous management of patients, observation of psycho/physical therapy sessions and close interaction with their families, primary care physicians, and clinic staff.

Resident expectations on elective:

This elective is offered to 1st, 2nd and 3rd year pediatric residents under the supervision of the Elizabeth Super, MD and Helen Turner, NP. It will be offered throughout the year. One resident is allowed per month and will be determined on a first-come, first-serve basis. Responsibilities include:

Sleep clinic:

- Independently seeing patients in sleep clinic and staffing with the attending; writing notes and obtaining feedback on note writing

- Discussing and critically evaluating sleep studies with the attending

- Reviewing common sleep disorders in children with the attending including obstructive sleep apnea, night terrors, nightmares, restless leg syndrome and narcolepsy

Pain/coping clinic:

- Evaluating patients in pain clinic with the multidisciplinary team (anesthesiology, psychology and physical therapy)

- Optional daytime rounding with inpatient pain team

- Observing psychotherapy sessions in coping clinic including biofeedback

- Observing and participating in physical therapy sessions with patients that have chronic pain
