Overview:

This is a combined inpatient and outpatient rotation through a prominent pediatric subspecialty.

Rotation goals and objectives:

1. Review basic microbiology (Preliminary identification by gram stain, blood, urine, and other culture methods, Sensitivity testing methods)

2. Be able to perform a complete evaluation of a patient with a possible infectious disease

3. Review antibiotic classes and appropriate use

4. Be able to perform evaluation and management of various specific infectious diseases

5. Review etiologic agents, clinical manifestations, diagnostic evaluation, and treatment options for congenital and perinatal infections

6. Understand the pediatrician’s role in prevention, counseling, and screening of infectious diseases

7. Understand initial assessment and management of patient with possible immunodeficiency

8. Learn to provide a consultation service that is effective and valuable to primary service

9. Learn to do literature review to provide evidence-based recommendations for ID consults

Prior to the start of the rotation:

1. Obtain sign-out from preceding resident

2. Review rotation objectives

3. Read through the ID examination

First day of the rotation:

-Contact the Peds ID attending on-call immediately after morning report.

-Know where the Peds ID textbook is (see “Resources” below).

-Contact Heather O'Mara (4-3305) to let her know your continuity clinic afternoons.

Resident Expectations:

PATIENT CARE

1. Inpatient rounds

  • Pre-rounding should be done PRIOR to morning report.

    • In most circumstances you should examine your patients independently prior to attending rounds and prepare an independent assessment and plans.

    • Rounding with attendings will be dependent on many factors, including the attending’s other commitments. Hence – rounding times will vary day to day.

    • Mid and late afternoon new consult requests are not uncommon, and generally are staffed that day.

2. Outpatient clinic. Each week, choose one of the following and let your inpatient attending know.

  • HIV-specific. Tuesday 1-4pm. DCH 7th floor, room 7440.

    • General outpatient ID or OPAT clinic. Wednesday 9-noon. DCH 7th floor, room 7357.

NOTES

1. Do NOT copy-forward notes from day to day.

2. Templates to use:

3. We strive to have consistent recommendations in the attending attestation and resident note. Notes initiated early during the day are to be updated with new results and/or recommendations before you send them for attending co-signature. Please feel welcome to contact your attending if you need any clarification at any time!

MICRO MORNING

1. You will be spending one morning (~2 hours) in the Micro section of the Core Lab (with Dr. Mick Scanlan) to gain appreciation for the testing done (PCRs, cultures, C diff, etc).

2. Be on the look-out for an appointment request in your Outlook calendar (If you have a student on rotation with you, they should accompany you for this experience)

PAGER

1. Expected hours for residents to be contacted for consult via pager will be M-F, 8a-5p. Smartweb will reflect this.

2. Outside of those times, please turn your pager off ("out of hospital") to avoid providers attempting to reach you.

3. If you are in continuity clinic between the hours of 8a and 5p, forward your pager to the attending until you are finished.

General Schedule:

830-9a: AM report

9a: call attending to determine rounding schedule.

Morning: pre-rounding, rounds with attending

Noon: go to resident noon conference

Afternoon: pre-rounding, rounds with attending

Exception:

Wed AM – ID clinic

Thurs Noon – Citywide Conference

If you are going to be absent for all or part of a day:

1. Notify the ID attending on that day, the chief residents, and Mike Grubbs

2. Any resident missing more than 2 days (or 4 half days) may be required to make up the difference at a future date at the discretion of the rotation director, chiefs, and program leadership.

1. You will be provided with a copy of the textbook edited by Sarah Long “Principles & Practice of Pediatric Infectious Diseases.” Obtain this copy from previous rotation residents/fellows, and then pass on to the next resident.
