Maternal Child Home Visit


Background:

During this experience, you will learn about the importance of home visiting programs with a focus on early childhood. According to the AAP, “High-quality home visiting services to infants and young children can improve family relationships, advance school readiness, reduce child maltreatment, improve maternal-infant health outcomes,

and increase family economic self-sufficiency. Home visiting refers to a strategy in which a professional or paraprofessional renders services in a community”.

Experience:

You will meet with a community health nurse, learn about home visits, and

observe nurse home visits.

Prep Work:

Please review the reflection questions, AAP policy statement, and websites below. These will provide you with the context about home visits and care coordination more broadly, as well as some “questions to consider” during this experience. Consider how care is experienced “Beyond the Medical Home” and the importance of effective communication among members of the health care team.

Parking is free. Wear Comfortable clothes. Located out in the field, so if it is hot, it is ok to wear sandals and 3/4 length pants. Bring a sack lunch and a microwave is on site if a resident wants to heat food.
