Elective description:

The main focus of this elective is to further train pediatric residents in the realm of pediatric hospital medicine (PHM) and its unique responsibilities. A study conducted among practicing hospitalists[1] suggests that these graduates felt less prepared in areas such as medical consultation, communication skills, systems issues and continuum of care competency. There is also little if any training in administrative responsibilities and quality improvement processes during standard residency training. Therefore, this elective will emphasize the development of three main areas in which a future pediatric hospitalist should be skillful: clinical, academic and administrative. The rotating resident will gain expertise in these skills through autonomous management of patients and close interaction with their families, primary care physicians, and hospital staff.

Resident expectations on elective:

This elective is offered to 3rd year pediatric residents under the supervision of the pediatric hospital medicine faculty. It will be offered during high season when the Hood service provides double covered weeks (i.e. ward 2) to maximize exposure. One resident is allowed per month and will be determined on a first-come, first-serve basis. The resident will serve primarily as a clinical “pre-tending” for the ward 2 Hood service, admitting patients from community practices, outside EDs or the OHSU emergency department to the ward 2 service. The patient assignment and load will be determined at the discretion of the primary Hood senior, who assigns patients to ward 1 and ward 2. The elective resident will function in the role of an attending by:

-Rounding independently with the intern and student. Patients will be staffed with the ward 2 attending after rounds.

-Documenting attending notes

-Completing mock billing for patient care

-Organizing and leading care conferences, as needed

-Taking admission calls through the transfer center, with supervision

-Communicating with PCPs

-Serving as the primary educator for the ward 2 service

In addition to the clinical hospitalist responsibilities, the resident will be invited to attend the quality and administrative meetings related to PHM, including: the Pediatric Acute Care-Medical steering committee, Doernbecher Quality Committee, and PHM division meeting. Residents will be provided training in hospital based billing, and will have the opportunity to participate in PHM related quality improvement efforts and educational activities, tailored to the individualized needs of the resident.

Schedule:

~7am-5pm in house with pager call til 9pm Monday-Friday (hours may vary, pending clinical schedule)

Elective description:

The purpose of this elective is to give a 3rd year pediatric resident an opportunity to hone their skills in inpatient pediatric medicine while gaining more insight into the responsibilities of a pediatric hospitalist. The main focus will be to give the resident more autonomy with inpatient management and communication with the hospital staff and families. The resident will also have the opportunity to participate in some of the administrative and educational duties of the pediatric hospitalists at our institution.

Resident expectations:

- The resident will work a combination of day shifts, night shifts and weekends totaling about 40 hours a week. The schedule will reflect that of a pediatric hospitalist at our institution.

- On day shifts the resident will be expected to round on 6-8 patients as a “pretending” attending and then report to the assigned pediatric hospitalist. The resident will be given more freedom to make patient care decisions independently. Rounding may or may not include interns and medical students.

- The resident (with supervision) will answer the admission phone and triage calls from the emergency department, primary care providers and outside hospitals.

- The resident will be expected to complete admissions on both day and night shift and report to the assigned pediatric hospitalist.

- The resident will give a 30 minute lecture to the rotating residents and medical students on a topic of their choice. The resident will also be expected to provide education during rounding and while completing admissions.

- The resident will be given the opportunity to attend our ward team meetings, peer review meetings, practice review meetings and participate in other hospital wide quality/operations committees.

Scheduling:

As stated above, the resident will be expected to work a combination of days, nights and weekends totally about 40 hours a week. The schedule will be very flexible and we should be able to accommodate all requests.
