PGY-1 Responsibilities

Clinical:

Overview: During the three rotations through the General Pediatric Clinic, the PGY-1 will evaluate and manage common medical problems seen in the ambulatory clinic setting.

This can be quite a high volume rotation! During the winter, as an intern, you may see 10-15 patients in a day. You should complete an entire history and physical exam, and formulate an assessment and plan of care, prior to presenting. Intern may start workup as appropriate (rapid strep, urinalysis).

Throughout the year, your time management skills in clinic will improve. Clinical histories will be focused, and presentations will be pertinent. Your skills as a general pediatrician will greatly improve after three rotations in clinic.

Schedule:

Morning conference attendance is expected on this rotation. You are expected to arrive in clinic promptly after morning report or grand rounds. The first patients are scheduled at 9 am.

The general pediatrics daytime hours are 9 am to 5 pm. Expect to stay in the afternoon until you have completed your last patient visit.

The general pediatrics evening hours are Monday and Wednesday from 5:30-8:30 pm.

Adolescent medicine evening hours are Monday, Tuesday, and Wednesday 5:30-8:30 pm.

Saturday and most Monday holidays, the clinic is open 9 am to 1 pm. Expect to stay until you have completed your last patient visit. This may be later than 1 pm if you are waiting for lab or Xray results.

Saturday and Monday holiday clinics are covered by either a general pediatrics or CACH intern. Check amino.com for the schedule.

Clinic A & C interns will also be taking 1 weekend per rotation of MBU shifts

If you have not yet rotated in the MBU, please plan to join the MBU intern in the MBU at 7am on the FRIDAY MORNING before your first weekend. During this time you’ll learn the logistics of how to pre-round and write notes on babies in the MBU. You’ll join the MBU team for the first few babies on rounds at 9am and will then return to clinic around 9:30 for the remainder of the day.

Clinic A intern present for entire rotation. Clinic B & C interns will have 1 week of vacation each.

Educational: There will be a pre-clinic conference for the first 10-15 minutes (patient load permitting), lead by a rotation of interns, third year resident or the attending. A list of topics will be on the bulletin board that should be covered during the month. Talks, articles and handouts can be found on the shared X drive: X:\SOM\PEDS\General Pediatric Clinic Talks. This is a resource meant to be shared and added to during your rotation.

Third year residents will be in charge of making sure topics are covered and assigning talks. In addition, the mornings can be a time to discuss follow up of patients, interesting films, or go over a Peds in Review Case.

Interns will have the opportunity to spend time with Kathy Warden, RN in triage to learn phone triage in preparation for taking parent advice calls as a second year resident.
