Early Intervention

Background, Contacts, and Location: Goal is to learn about the EI process from provider referral to intake to actual evaluation, in order for the provider to be better able to prep families and increase engagement. Also hope residents become familiar and learn to use the EI Universal Referral Form, which will allow for more efficient intake and contact of families and for more successful communication back to referring provider, and which also includes a physician statement that allows providers to refer kids early, before they show any delay (low birth weight, congenital issues, etc).


*Please note that Oregon has one of the strictest eligibility criteria in the country, so there will be some children referred to EI who aren’t eligible for EI services but still need support services. For those patients, and for those patients that EI is not able to contact or get in for evaluation, getting communication back from EI and learning how to act on the information provided is key.

*After turning onto Ray Circle, turn right into the second driveway. Follow it back until you see a red building on your left it will have the numbers 5825 on it.

*Note that Hwy 26 can be very busy during the AM commute, so it is good to leave early.


Experience: AM or PM evaluation of referred families. Assessments start at 0900 or 1200, but please arrive at 0830. Residents will observe an assessment/evaluation either on site or at a home visit and hear more about the services available to children and families from the Early Intervention program. Residents will meet with one of many potential staff.
