STEPS:

1. Clients seated and offered a beverage

2. Client given a care guarantee form and a pen to fill in

3. The client only fills in the white section

4. Senior consults once the form is filled in

5. Senior fills in the grey section

6. Once completed at the end of service, the form is scanned and kept in the client profile
