It is only under very specific circumstances that a "reasonable suspicion" drug or alcohol test would be ordered for an employee, but on occasion it is the right thing to do. If an employee is impaired at work, steps should be taken to protect the employee's rights, the safety of other employees and the public, and the employer. The form below may be used by a supervisor or manager as part of a process in close coordination with the Human Resources Department.
