A Performance Review is a formal assessment of your performance usually assessed by a few Key Performance Indicators dated between the date of your current review and your last review.
When completed correctly, a performance review should:
Ensure an annual platform for open discussion about workload and performance is available for both the employee and the employer
Highlight the "wins" over the past period.
Discuss areas that require improvement
Provide a clear understanding of the roles and responsibilities
A performance review is made up of the following steps:
Set a meeting date. Let them know you are coming in to conduct their performance review on "Date" at "Time".
The meeting should be off site. It can still be in a formal setting should you choose however it needs to be away from and disturbances where other staff or members cannot pop in to say hi or ask for help. This is especially important if your team member has had a rough time or seems off. You both will appreciate the privacy for some radical candor or freedom of expression if needed.

The Questionnaire. After a discussion of the KPI's it's time to complete the Questionnaire. With the results of the last period in font of them, use the questionnaire to see how the employee feels about each topic. Give them the time to be heard.
Throughout this process it is important to remain aware of the employee's responses, are they above or below the line?
