*For all chit-chat/interview sessions, Interview Forms must be filled up with the basic information of the interviewees.

- Tutorial Services
- SuperTouch
- Food Provision
- Health Care e.g. eye-check, glasses
- Laundry Services
- Home Improvement e.g. from electrical products to repair of house
- Financial Aid e.g. Payment of Debt / Rental / Utility Bills / School Expenditures
*J&M Forms: Rebekah (Chinese) & Persis (Indian) to follow up with Ms Tracy's advice and approval.
(b) Application of services that require to fill up ONLY the Heart-to-Heart Interview Forms
- Love on Wheel
- Blessing Bags
- Dialysis Centre
- Government aid
- Job placement (Not handling)
- Counseling (Not handling)
*Interview Forms: Yue Shan to process (including filing of the forms).
Service Request Book
- From the Interview Forms, have the specific request / recommendation of services written in the Service Request Book.
- From there, Yue Shan will follow up on what are written in the Book with Ms Tracy's advice and approval.
- In cases when a J&M Form has to be filled, Rebekah is to make appointment with the applicant.
