Once you have learned the requirements for home schooling in your state you can use the following forms to customize your teacher's record book. Many of the forms are useful, even if they are not required by your state. The Home School Legal Defense Association recommends keeping good records even in states without strict requirements. As a minimum, they recommend keeping:
1. Report Cards
2. All standardized test results
3. Immunization records and any medical records which relate to education.
4. A portfolio which includes samples of your child's work in each area of study.
5. All tests you give your child
6. Records of Field Trips and related activities
7. A log which records a summary of daily activities
8. A signed statement of your family's religious conviction about home schooling (if applicable)
These forms and many others are included. A summary of daily activities can be documented from assignment sheets or scheduling sheets. Note that not all families use tests in their schools. However if you give tests to your children, the form for recording test scores will help you track the scores.
Most of the forms are left blank so that the teacher can customize them with the names of students, months of school, and subjects. Examples of subjects that could be studied are: Literature, Grammar, Vocabulary, Spelling, Mathematics, Handwriting, Writing, Foreign Languages, Science, Social Studies, History, Bible, Courtesy, Unit Studies, Touch Typing, Music Theory, Art, and more! A Goals Work Sheet is provided so that you can prioritize your students' studies. It is best to start simply. Do not try to do everything at once!
Many parents are concerned about the requirement of a portfolio. The portfolio that you assemble can be as simple as a folder for each child that contains periodic samples of work in each subject area. Remember to put the child's name and date on the paper, if it is not already labeled.
