Move-In Program Students

In-State Move In Students Process:

  1. Student registers at neighborhood school

  2. Student attends neighborhood school

  3. Neighborhood School request records from prior school

  4. Notify your Support Specialist if the student was placed in a "program" or specialized placement immediately.

    1. Support Specialist will take student through Program Review, and communicate with you.

  5. Neighborhood school is responsible for the following paperwork and paperwork distribution to the parents and the AC(via Google Form):

    • Signed Initial Provision of Special Education

    • IEP packet (IEP, Placement, PWN, Meeting minutes)

    • Signed Eligibility Packets and Supporting Documentation

  1. Neighborhood School brings the file into compliance prior to change in location/placement

Out of State Move-In's into a Program Process: Neighborhood School's Responsibility:

  1. Student registers at neighborhood school

  2. Request records from other state

  3. Notify your Student Service Support Specialist if prior placement was in a "program" or other restrictive setting

  4. Student attends neighborhood school

  5. IEP and Eligibility Requirements all move-in's from other states/countries:

    • Consent for Evaluation and Evaluation Planning

    • Determination of Oregon Eligibility

      • Establish Oregon Eligibility with a new date since enrollment in HSD

    • Hold an Oregon IEP Meeting and determine Placement

      • Oregon IEP must have new date, the date from the other state is not valid

    • Signed Initial Provision of Sped Services

    • Distribute Paperwork AC via the google form

      • ALL Oregon Paperwork

      • Other State's IEP and Eligibility Paperwork

If the student is a placed into a program in HSD:
