Assessment Tools
Assessments are to be listed on the Consent for Evaluation, following evaluation planning with the team (2 Professionals and the Parent), always involve the staff member that will be completing the evaluation in the evaluation planning process. Use the information for specific tests below and on the related pages to provide informed consent to parents. List the specific Assessments on the consent for evaluation.
Informed Consent Considerations:
New testing can be conducted only if the parent has given informed written consent for the evaluation with documented evaluation planning meeting minutes
Case Manager must Explain each test requested
Consent for Evaluation must list the tests to be used, and signed by the parent
Verbal consent is never allowed
Developmental History (English) (Spanish)
Description: Information about prenatal and early childhood health and development
Summary: Information gathered from medical records and parent reports concerning the student’s prenatal and early childhood health and development.
File Review
Description: Review of Records
Summary: An analysis of educationally relevant records available in the cumulative file, teacher data, and may include the following:
Attendance Records
Classroom Assessments or Interim Assessments
Discipline History
English Language Proficiency
Health History - Vision and Hearing
Statewide Assessments
Academic and Functional interventions
Interviews
Description: Interview to obtain information about factors pertaining to school success
Summary: Structured or semi-structured interview with parents, staff, student, or other people knowledgeable about student and factors pertaining to student’s ability to succeed in school.
Medical Statement - Use form in ORSPED
Description: Statement of whether health or sensory factors may be impacting learning
Summary: A statement from a medical practitioner in answer to the question of whether health or sensory factors may be affecting learning
Observations
Description: Observation of the student in an educational or other setting
Summary: Students are observed by a Special Education Teacher for purposes of establishing an eligibility, and a report is written documenting the student observation. Observations used for determining eligibility can not be completed by an SEA, or General Education Teacher.
