Private Schools
~ Service Plan ~

A Service plan is for students who are attending an ODE Accreditted Private School, and are requesting services from the private school.

Guidelines for Service Plan

Provide parents with a copy of their Procedural Rights.

Complete all demographic information.

The Service Plan Team should include:

• The parent(s) of the student

• A private school teacher or principal

• A special education teacher or provider A representative of the district who:

  • Is qualified to provide, or supervise special education

  • Is knowledgeable about the general curriculum

  • Is knowledgeable about the availability of services to private school students (if the team anticipates the consideration of services requiring resources beyond the school’s control, invite a Special Programs’ administrator

Complete the Services Summary:

• A clear description of the services being provided

• The anticipated amount, location, and frequency of the service (this will be somewhat determined by the fact that cost of service must stay within the amount allocated per private school child; this is recalculated annually)

• The projected dates, initiation and duration of services

• The provider of the services

Determine the extent of non-participation with regular education students and provide an appropriate explanation.

Describe the student’s Present Level of Performance:

• Provide a clear picture of the student’s strengths and needs as determined through evaluation

• Identify how the student’s disability(ies) affects his/her involvement and progress in the general curriculum

• Include the student’s strengths

• Include the concerns of the parent regarding the student’s education

From the Present Level of Performance develop the Measurable Annual Goal. The annual goal must be directly tied to the needs addressed in the present level of performance and must be written in measurable terms including:

• How successfully meeting the goal will be demonstrated

• The criteria to be used

• The evaluation procedure that will be used

• Include intermediate two short-term objectives that will move the student toward accomplishing the annual goal

Describe any accommodation and related aids and services that will be necessary for the student to meet his Services Plan goals and participate in the private schools general curriculum.
