Progress Notes

The IEP must include a description the child's progress towards the students Annual Academic and Functional Goals:

    • How the progress will be measured

      • Written progress

    • How the progress will be reported, including the frequency

      • Written

      • With general educaton report cards

    • Data tracking sheets are available in OrSped to help you with tracking student's progress

Progress must be both narrative and supporting data for each annual measurable goal be specific and listed in measurable terms that relate to the annual goal.

    • Progress notes must be narrative and data

      • For example

        • Student A has mastered 5 of 10 sounds/words/math facts for the current grading period, and is working hard on gaining mastery of the words/sounds/math facts.

        • Sudent is comprehending grade level texts with 80% accuracy as demonstrated by the most recent classroom assessments and completed a book report on the book.

Case Managers Responsibility:

    • Gathers data and writes progress notes for the goals that they provide specially designed instruction

      • Speech Language Pathologists and other Related service providers(APE/OT/PT) will enter their progress notes into OrSped

      • Goals where two or more providers support the goal, progress is collaboratively reported in orsped

    • Case Manager reviews all progress notes in OrSped prior to distribution and verifies that progress has been reported for each and every goal each grading period.

      • If there is progress missing for a specific goal it is the case managers responsibility to contact the related service provider and have progress reported

    • For students who have multiple special education providers, it is the case managers responsibility to make sure that progress has been reported accurately in OrSped.

Distribution of Progress Notes:

    • Parents are sent quarterly progress reports with report cards

    • AC is sent students' progress for the proceeding 12 months with the annual IEP

    • Case managers are responsible for collecting and distribution of all progress reports

    • Progress reports are filed in the CUM file each grading period
