The district must ensure that the child’s IEP Team is informed of these changes and copies are distributed.
An IEP amendment is appropriate to address:
Lack of expected progress towards the annual goals
If there is a reevaluation conducted(e.g., AT, OT, PT, APE or addition of an Eligibility)
Team gathers additional information about the student
The students needs change
Student moves into Hillsboro with another Oregon IEP
Implementation of a BSP/FBA
Other

Revise(Cover Page with amendment date; keeping annual date the same, and page with Errors)
Write Prior Notice of Special Education Action and Distribute
Revising Goals:
Send meeting notice to team
Hold meeting with Team including the parent and teacher
Revise(Cover Page with amendment date; keeping annual IEP date the same, Goals, Present Level, Service Summary if applicable)
Write a Prior Notice of Special Education Action
Distribute all pages you revise
Document Team Meeting in meeting minutes

Hold meeting with Team including the parent and teacher could be completed via phone
