Functional Behavior Assessment
Behavior Support Plan
Any student can have a FBA and BSP completed, for a general education student who is not in the special education referral process work with your counselor and obtain the consent through the counselor.
Gen Ed/504 Consent to Evaluate - FBA (English) (Spanish)
Special Education students who are in referral or have a current eligibility, use the consent in ORsped and attach the meeting minutes to the signed consent for evaluation.

New Functional Behavior Assessments and Behavior Support Plans require a Signed Consent for Evaluation and Evaluation Planning Meeting Minutes and best practice is to send a meeting notice for the FBA/BSP meeting in advance of the meeting.
Consent for Evaluation:
FBA/BSP requires a Signed Consent for Evaluation and Evaluation planning meeting minutes
A revision to the BSP if the function of the Behavior is the same doesn’t require a new consent for evaluation
A new consent for evaluation is needed if the team suspects that the function of the behavior is different or the student is in a new environment/school.
Meeting Notice:
A meeting notice is appropriate for both the evaluation planning to complete the FBA/BSP.
Meeting minutes are appropriate during the BSP meeting
Considerations:
The Behavior Box on the considerations box would be checked yes for all students with a BSP.
ODE Guidance:
The IDEA strongly supports the notion that IEP teams should proactively consider and implement BSPs (Behavior Support Plans) based on functions of the student’s behavior (i.e., not waiting for a removal situation).
Districts are required to address behavior that impedes the student’s learning or the learning of others.
If Yes, the IEP team must describe Behavior needs in the PLAAFP; needs need to be addressed in the following areas: service summary, goals, assessment, ESY and/or transition
IEP:
An IEP revision (amendment) is needed if a BSP is added for the student, the Behaviors would be written in the Present Levels of Functional Performance. This section should accurately paint a picture of the student's functional behavior (Data and Narrative). This information should be updated anytime a BSP is changed, document in the present levels of Functional Performance with data and narrative.

“student has needs in the area of social interaction with peers and controlling outbursts in class when situations are out of his control…” (Statement would then be supported by data)”
“…a functional behavioral assessment (FBA) was conducted on 10/12/2013 and a behavior intervention plan (BSP) was created to support the student in addressing self-stimulation concerns…(Statement would then be supported by data)””

Does the student need a behavior goal? Team decision
Service Summary Page:
List the Behavior Support Plan as an accommodation on the service summary page.
HSD Policy requires the Behavior Support Plan be uploaded to Orsped and distributed to all staff in contact with this student.
