Evaluation Components for Eligibility under the Category Other Health Impairment

1. The team has reviewed existing information, including information from the parent(s), the student’s cumulative records, and previous individualized education programs or individualized family service plans. Evaluation documentation includes relevant information from these sources used in the eligibility determination

    • This can take the form of a file review (see district file review form), developmental history, medical reports, psycho-educational evaluation reports, Statewide Assessments, and/or meeting minutes

2. A medical statement or a health assessment statement indicating a diagnosis of health impairment or a description of the impairment, and that the child’s condition is permanent or is expected to last for more than 60 days

    • Needs to come from a medical doctor, physician’s assistant, or nurse practitioner

    • Can be a diagnosis or description of the impairment

3. Assessments to determine the impact of the suspected disability

    • Educational impact can refer to difficulty in the areas of academic skills, academic performance, social competence, emotional/behavioral regulation, attendance, stamina

    • Assessment tools to measure adverse impact may include:

        • Standardized or informal assessments of academic achievement

        • Behavioral rating scales

        • Observations

        • Interviews

4. Additional assessments that are necessary to identify the student’s educational needs

    • Other assessment tools that may or may not be directly related to the suspected disability and further assist with identifying a student’s educational needs

Once these evaluation components have been considered by the eligibility team, the team must determine whether the student needs special education services (specially designed instruction), accommodations through a Section 504 Plan, or neither.
