SLD Eligibility Observation
Observation Requirements:
You can use the prior observation if it is still relevant and applicable to the student and the observation was completed by a special education teacher
The observation report must be attached to the statement of eligibility as supporting documentation
If you do not have the report of the observation or if it is not relevant currently
Generate and obtain a written consent for the observation
Conduct the observation and attach the report to the statement of eligibility
Observation must be completed by a special education provider, SEA's are not allowed to observe for eligibility determination
