Evaluation Planning Documentation
The following must be documented input from two professionals and the parent, you can choose to document in meeting minutes or on the document provided(below) by ODE. The Consent for Evaluation and the Evaluation Planning documentation must be stapled together and distributed.
Most recent testing and evaluations
Information provided by parents
Current classroom-based, local or state assessments;
Classroom-based observations; and observations by teachers and related services providers.
Staff who will be conducting the evaluations must be included with the evaluation planning process, and their input documented in the evaluation planning documentation.
For an ASD evaluation include your ASD consultant
For an Assistive Technology referral include the AT staff
For an most eligibilities you will need to include your school psychologist
For a CD eligibility include your SLP
For Vision include the Vision Specialist
For Hearing include the Hearing Specialist
You should always include the general education teacher
The evaluation must be:
Comprehensive, and conducted in all areas of suspected disability(ies) and identified all the student’s special education and related services needs, whether or not commonly linked to the disability(ies);
Use a variety of assessment tools and strategies to gather relevant functional, developmental and academic information, including information from parents to assist in determining whether the child is a child with a disability and the content of the child’s IEP, including Information related to enabling the child to be involved and progress in the general curriculum; and,
Not use any single measure or assessment as sole criterion for determining a disability and appropriate educational program. §300.304
