Nursing Note, October 2022
Patient arrived at 1038 for his appointment at 1030, accompanied by his social worker. At 1040 I visited pt in lobby to let him know that he was late, that we would see him when possible, and that it would be about a 10-minute wait. Pt was immediately visibly upset - pale skin, clenched fists, and hunched over in his chair. Pt said he was furious. Pt exhibited some unnerving anger, such as with Autism diagnosis: shaing hands, eyes shifting, open mouth. I approached pt and spoke softly, saying that I was really sorry, and that he was right - just because he was late, didn’t mean that he should have to wait longer - but that Wednesdays are our busy clinic days, and that I would get him in as soon as possible. He seemed calm. At 1045 I returned and called him back to his pt room. He preferred to comduct the visit without his social worker. He continued to be calm. Upon rooming pt, he became upset again because he didn’t like having to answer so many questions. Pt had slight speech impediment. I apologized and said that I had to go fetch a thermometer. I spoke with his social worker, and asked for any tools or advice, he had a list of medications and confirmed that he was autistic. I verified the medications in his chart and that he was diagnosed with Autism and had a criminal history. When I returned to pt room, I finished rooming, and he apologized for being so mad. He agreed with me that he was scared because he didn’t know why he had to have this appt. We discussed eating healthy and maintaining his weight. At 1053 I spoke with the provider, alerting him of pt concerns. Vitals were normal, BP slightly elevated. Upon discharge, <lead MA> requested that I do the follow-up, but he was able to complete it with instruction on working with an Autistic pt, such as being chatty, positive, empathetic, and a good listener.

One of the many projects I managed while at Engineers Without Borders (EWB) was a clean-water-supply project in San Antonio Abad, San Salvador, El Salvador, through Colorado State University, in which we considered: the cost of drilling a well, infrastructure for the well, a water pump, distribution pipe, a storage tank, a filtration system, 100-year system maintenance, and travel for our assessment and implementation trips. Strategic planning was required for funding the project through fundraising, pro bono services, donations, and potential buy-in from the village. The Peace Corps volunteer on the ground helped us to research a drilling company, materials for well infrastructure, pipe, and storage tanks. The water pump we purchased from a company in Germany. A filtration system popular among EWB projects at the time was tilapia fish tanks, which also help with local economy and food supply. Maintenance included flood analysis and degradation of all systems. For travel, we included food, lodging, airfare, ground transportation, required vaccinations, medications, and first-aid supplies. We carefully analyzed and asked for labor and financial buy-in from the village to (1) ensure that they would take care of the system, (2) help pay for the system, and (3) promote education about the system. Fundraising included two galas, selling t-shirts, and a 5K run on campus. A professional engineer who helped with mockup designs, the geologic survey, and updated designs, and a nurse that accompanied us during travel and assisted with our health surveys, were the pro bono contributions. We worked with the local rotary club to secure some donations. Business operations with CSU, EWB, Peace Corps, and the rotary club involved project proposals, updated budgets, reports on the village and our geologic survey, and all financial operations.
