• Notarized Statement of Identity and Educational Purpose form with a copy of ID that was seen by notary 

• Signed tax returns or tax transcripts

• Documentation for Appeals (Satisfactory academic progress [SAP] or Unusual Enrollment History [UEH])

• Documentation for Professional Judgments

• Documentation for Dependency overrides

• Documentation for Independent Minor

• High school diploma or high school transcripts with graduation date

• Dependent without Parent Data form

• Family Size form

• Overlapping Loan/Aid Clearance letter 

Important: 

• Ensure your Student ID Number is included on all forms/document

• All forms must be fully completed and wet signed (original signatures only) 

Please DO NOT submit the documents listed below in the drop box. These documents require in-person submission. Financial Aid staff must review the original versions of these documents. Please schedule an in-person appointment to submit them.
