Falthar
In order to qualify for Loremaster, Falthar should be at least 7th level in Diviner, in order to have 10 ranks in two Knowledge skills (rank limit = level+3). The simplest fix is simply to raise him to Diviner 7th, which results in the following changes: +1 to Atk & all saves; +2d4+4 HP; +1 feat; +12 skill points; gains 5th-level spells (Spells per day now 4/5+1/5+1/4+1/4+1/2+1).
His spell list (for Div5/Lor3) includes either his specialist bonus spells or his Int bonus spells, but not both; add one spell per spell level. This list also lists too many cantrips; trim it to 4. If raising his level as suggested above, add additional spells as appropriate; I would suggest contact other plane, hold monster and sending for his 5th-level spells. Falthar's prohibited school is not listed, but from the sample spell list, it must be either Conjuration, Illusion or Necromancy (most likely the last, given his aasimar nature). Assuming Falthar is raised to caster level 10, change his spell rates to 120, 240, 360, and 480 gp, and add "5th level--600 gp". (The previous rates were calculated as if Falthar was 2 caster levels higher.)
Wands may only hold 50 charges (not 53!). [Tales corrected this to 40.]
Nell
Weapon Focus should be Weapon Finesse. Bards do not prepare spells, so treat her list as "Spells Known," but delete one from each spell level. Nell knows 5/2, and casts 3/1 per day.

Falthar appears in the d20 Freeport Companion. However, by the time of The Pirate's Guide to Freeport, he has lost Nell.
