Chapter 1: Hair pulling and skin picking disorders: The basics
This chapter contains: A description of body focused repetitive behavior disorders (BFRB); an overview of theories about what causes BFRB, including biological and learning theories; and a description of treatment approaches, including the Comprehensive Behavioral Model (ComB).
(2) Preparing for change
This chapter discusses how to overcome obstacles to engaging in treatment of BFRB, including dealing with secrets and shame and unhelpful beliefs.
(3) Ready, Set, Go!
This chapter provides a complete overview of the Comprehensive Behavioral Model (ComB) for treating BFRB disorders. There are three basic stages to ComB:
Stage 1: Building your awareness: In this stage, you identify the triggers and consequences of engaging in BFRBs. An essential aspect of this stage is using a Self-Monitoring Form (which can be downloaded here).
Stage 2: Planning and preparation: In this stage you will identify and prepare to use promising strategies to address the triggers and consequences you identified in Stage 1. There are 5 domains that the strategies address (sensory, cognitive, affective, motor, and place) and these are covered in Chapters 4 to 8 of the book. A master list of strategies can be downloaded here.
Stage 3: Putting your plan into action: In this stage, you implement the strategies you identified in Stage 2 and monitor their effect on your behavior. An essential tool for this stage is the Action Plan Form, which can be downloaded here.
(4) The sensory domain
In this domain, you will focus on finding substitutes and activities that divert you from picking and pulling by stimulating or soothing your senses.
(5) The cognitive domain
In this domain, you will be encouraged to challenge the thoughts and beliefs that encoruage you to pick or pull. You will learn to replace or counter them with more realistic or helpful thoughts.
(6) The affective domain
In this domain, you will be encouraged to discover and employ healthy methods of managing your emotions and other uncomfortable feelings, instead of picking or pulling.
(7) The motor domain
Here, you will focus on identifying and addressing any unconscious habits, movements, and body positions that encourage your picking and pulling. You will increase your awareness through the use of barriers, for example, that impede or prevent contact between your hands and hair or skin, and by providing alternative ways to keep your hands busy and away from your picking and pulling sites.
(8) The place domain
The place domain includes all aspects of your environment or setting (for instance, time of day, situation, or objects within your physical environment) that set the stage for picking and pulling. The interventions in this chapter will serve to decrease the power of these cues.
(9) Putting it all together
This chapter provides guidance in putting all of the strategies you have identified into a comprehensive plan.
