(1) Via my secure file upload page
The fastest and easiest method is to use my file upload page at the link below. They can be documents (e.g., .doc or .pdf) or photos (e.g., jpg or png).

You can also send documents to me via the Patient Portal, although it is not as easy as the secure file upload page. Go to: https://www.corvallisclinic.com/portal

(5) How to scan documents with a smartphone
If you have only one or two pages to scan, you can simply take a photo with your smartphone, and send those to me.
If you have multiple pages, I recommend downloading and using the free Adobe Scan app to create scanned documents with your phone's camera. Here are step-by-step instructions:
(1) Download the free Adobe Scan app to your smartphone.
(2) If you don’t already have an Adobe account, you will need to create one. This is free.
(3) Scan the document with your phone’s camera. It will save as a PDF.
(4) Choose, “Share a copy” and email the document to yourself.
(5) Go to my secure file upload page to send the document to me (see address above).
