Utilize Surveys in Brightspace to solicit feedback from learners regarding any aspect of a course. Surveys can gather anonymous or non-anonymous opinions and information. Unlike the Quiz tool, Surveys do not require right or wrong answers and can include Likert-style rating questions.
Use an informal poll to ask your learners about their backgrounds, previous education, and interests in the course. You can choose to make the survey anonymous or share other learners' responses. Here are some example questions that you can add to the survey:
Short Answer: "What do you know and want to know about [subject]?"
Multi-Short Answer: "Tell me about 3 hobbies that you have and what you enjoy most about them"
Multi-Select: "What Brightspace tools have you used? Select all that apply."
Multiple Choice: "Select the statement you identify with most about [subject]."
Use an informal course evaluation to see where your learners are at halfway through your course. Here are some example questions that you can add to the survey:
Likert: "On a scale of 1-5, tell me how comfortable you are with [subject]."
Written Response: "Tell me how you are currently feeling about your performance in the course and what I could do better."
Multi-Short Answer: "Give me 2 examples of how you are using what you learned."
Use an informal reflection survey at the end of a Brightspace Module to gauge how learners are doing and what they learned. Here are some example questions that you can add to the survey:
Likert: "On a scale of 1-10, rate your confidence level with what you learned in [module]."
Ordering: "Place the following items in order from the most interesting to least interesting."
True or False: "I am ready to move on to the next module."
Written Response or Multi-Short Answer: "Tell me what was something you enjoyed learning and what you may have struggled with in this [module]."
In your course, navigate to the Module in your Content where you would like to add the Survey
Select "Upload/Create"
Click "New Survey"
Enter a title and instructions for the Survey
Uncheck the checkbox next to "Hide Survey from Users" so that the Survey will be visible to your Learners right away, or you can make the Survey visible once all of the questions have been added.
Click "Save" to create the Survey
Select the "Survey Setup" button to add questions
Under the "Properties" tab, find the "Survey Questions" section
Click the "Add/Edit Questions" button to start adding questions to your Survey
Select the "New" button to select a question type to add
If you already have an existing Survey, you may also use the "Import" button to add existing questions
Once a question type is selected, enter the question text similar to filling out a quiz question and click the "Save" button to add the question to your Survey
You may also select "Save and New" to add more questions without having to return to the Question Library
After all of your questions have been added, click the "Done Editing Questions" button
Adjust any other settings as needed and click "Save and Close" when done
Note: After you have finished adding questions and saving your survey, you may click the "Preview" button to preview the survey from the learner's view.
Properties Tab
Anonymous: Allows learners to submit the survey anonymously
Questions Per Page/Paging: Set the number of questions you would like per page and if you would like to prevent moving backward
Edit Values: Allows you to set if a question is mandatory or not
Invite Participants: Allows you to invite individual learners to participate in the survey
Restrictions
Attempts Allowed: Allows you to set how many times a learner can submit the survey
Unlimited: Learners can answer and submit a survey as many times as they want
Single attempt that is editable: Allows learners a single survey attempt. They can re-access and edit responses as long as the survey is still available
Limited: Sets a number of permissible survey attempts
Special Access: Allows you to give learners special access similar to a Quiz or Assignment
Reports Setup
Add Report: Create a report to download later to view all learner responses (Note: Set Role to Instructor/Developer)
Survey Branching presents questions one at a time and allows you to specify questions that learners will encounter after certain responses are chosen. You can set up Survey Branching by doing the following:
Navigate to your Survey and click "Survey Setup"
Scroll down to the questions area and select "Branching Wizard"
Note: Adding branching to a survey will remove the ability to add or edit questions, remove the current page layout and set it to one question per page, disable order shuffling, and remove the mandatory status from questions. A survey cannot be unbranched.
Choose the answers you wish to create a branch from and fill in the appropriate branching information
Click "Save" when done
In your course, go to More Tools, More..., and then Surveys
Click on the survey you wish to add your questions to
Scroll down and click the "Add/Edit Questions" button
Select the "Import" button and click "Browse Question Library"
Note: You will see questions from other Surveys and Quizzes that can be imported
Click on the "Source" dropdown menu to select the specific survey you wish to pull questions from
Select the checkbox next to the questions you want to add and click "Import" when done
Survey results can be viewed either by setting up a report, or by viewing the survey's statistics.
In your course, go to "More Tools" and then "More..."
Select "Surveys"
Click on the down arrow next to the survey with the results you wish to view
Select "Statistics"
From there, click on a student's name to view their submission. Please note that for anonymous surveys, attempts will all be together.
Survey reports gather information on survey data. Survey reports are different from survey results; they collect and present more types of information than is available through results.
In your course, navigate to your survey in Content
Note: If your survey is not in Content, go to More Tools, More..., and then Surveys
Click "Survey Setup"
Select the "Reports Setup" tab
Click "Add Report"
Enter a name for the report
Select which option you would like for the Report Type
Summary Report: You can display aggregate data for multiple choice, true and false, Likert, multi-select, and matching question types. You can also display text responses for written response, short answer, and fill in the blanks question types
Individual Attempts: This report displays each attempt individually. You can choose to hide the user’s first and last name when the Individual Attempts report is viewed
Choose when you would like the report to be released
Immediately: Allows you to download the report at any time
Set Date: Allows you to set a date and time when the report will be available for you to download
Select "Instructor/Developer" and any other role you would like the report to be available to
Note: If you need assistance from the DIID team with your report, select "System Admin (CE)" and "Sub Admin (CE)". Otherwise, support will need to impersonate your account to see the report.
In your course, go to More Tools, More..., and then Surveys
Select the down arrow next to the survey you wish to download the report from
Click "Reports"
Select the report's title
Choose one of the options to download a CSV, Excel, or HTML file
Note: If you allow students to submit multiple times throughout the course, you can use the "For attempts completed" to set a time range for the results using the "From" and "To" fields.
From there, you may open the report to view the results.

Click on the survey you wish to add your questions to

An anonymous survey may be a good way to gather honest feedback from your learners as they will be less afraid of their name being attached to the results.
