Navigation & Getting Started

You are able to start a form by:

  • Going to your Drive and selecting "New" > "Google Forms".

  • Selecting the app from the app launcher.

  • Going to forms.google.com

When a Form is started, it is automatically saved in your Google Drive!

This is a screenshot of the Google Forms landing page and the options for starting a Google Form.  The options are "Blank" where you start a classic form, "Blank Quiz", where you start a quiz where most questions can be auto-graded and you can enter feedback based on answers, "Exit Ticket", "Assessment", "Worksheet", and "Course Evaluation".  The last four options are templates for teachers so they don't have to start from scratch!

Once a Form is created, you will see the Form editing window where you can begin creating and adding questions!

TIP: If you wish to know the names of users who are answering your Form questions, it is recommended to have questions for first and last name. Otherwise, the most the Form could collect would be email address, which is difficult to determine who the respondent is.

Settings & Options

  • The Add-Ons button (puzzle piece icon) appears when a user installs their first Google Forms add-on. Clicking the button provides access to any add-ons that have been installed in Google Forms.

  • Select the Customize Theme button (palette icon) to access options to personalize your Google Form.

        • Change the header with pre-selected options from Google, or with your own custom image.

        • Alter the overall theme colors and background colors from a variety of palette options to make your Form stand out.

        • Alter the font style to add a more creative flair!

  • Select the Preview button (eye icon) to see what your Form would look like to your respondents. You can even take the Form yourself, making sure all questions, options, and settings are in place!

Click Settings (gear icon) to alter your overall Form settings. Popular settings include:

  • Collect Email Addresses

  • Restrict to users in CCPS and its trusted organizations

  • Limit to 1 response

  • Allow respondents to edit after submitting

  • Making the Form a Quiz

Click More Options (vertical ellipses) to access features like:

  • Make a Copy

  • Add Collaborators

  • Get Pre-Filled Link

Creating Questions

Once your form is set up, you can begin adding questions.

  • Click the add question button to start a question tile. You can also import questions from previously created forms!

  • Click the title and description button to insert text within your form. This could be additional directions or content for questions or just a motivational sentence!

  • Click the add image button to place pictures in your form. GIF's can also be inserted!

  • Click the video button to embed a YouTube video into your form. If using the form with students, students must be older than 12 and have parent permission for them to watch the videos.

  • Break your form apart in organized sections by using the add section button. This also allows forms to be "branched" where you can have respondents go to specific sections based on answers to their questions.

Question Editor

Question Types

Once a new question is started, a question editor tile is added to the form.

Teachers can change the question type from one of 12 options:

Short Answer

  • Respondents can write answers in a few words.

  • You can use the Equatio extension integration to type math problems!

  • You can set rules that respondents have to follow using "response validation" under question options

Number: ie. A whole number between 21 and 42.

Text: ie. Make sure that answers contain the word "candy," or are formatted as an email address or URL.

Length: ie. Limit answers to 500 characters or require at least 200 characters.

Regular expression: ie. Require text answers to match certain formulas that you choose.

Paragraph

  • Respondents can write long answers over one or more paragraphs.

      • You can use the Equatio extension integration to type math problems!

  • You can set rules that respondents have to follow using "response validation" under question options (see above).

Multiple choice

  • You can use pictures as answer options.

  • You can use the Equatio extension integration to type mathematical answers.

  • Respondents can choose between a set of options.

  • Respondents can only choose one option.

      • You can shuffle the order of options for respondents.

  • You can include "Other" as an option, and people can type a short answer.

  • Based on a person’s answers, you can send them to a certain section of the form using branching.

Checkboxes

  • You can use pictures as answer options.

  • You can use the Equatio extension integration to type mathematical answers.Respondents can choose between a set of options.

  • Respondents can choose more than one option.

  • You can include "Other" as an option, and respondents can type a short answer.

  • You can shuffle the order of options for respondents.

  • Based on a respondent's answers, you can send them to a certain section of the form using branching.

  • You can set rules that respondents have to follow using "data validation" under question options (see above).

Dropdown

  • Respondents can choose between a set of options.

  • Respondents can only choose one option.

  • Based on a person’s answers, you can send them to a certain section of the form using branching.

File Upload

  • People can upload files as a response to a question:

      • Uploaded files will use Google Drive storage space for the survey owner.

          • Uploaded files will be stored in a new folder.

      • You can choose the maximum file size that respondents can upload.

      • You can choose the maximum number of files that respondents can upload.

      • You can specify which file types respondents can upload.

      • To fill out this question, respondents need to be signed in to a Google Account.

Linear Scales

  • Respondents can rate your question on a scale.

  • Your scale can start at 0 or 1.

  • Your scale can end on a whole number from 2 to 10.

  • You can set a label for each end of the scale.

Multiple Choice Grid

  • You can create a grid where respondents can select one answer per row.

  • You can shuffle the row order for respondents.

  • To limit respondents to one choice per column, at the bottom right of the question, click "More" (three dots).

  • To shuffle the row order, at the bottom right of the question, click "More" (three dots).

Checkbox Grid

  • You can create a grid where respondents can select one or more answers per row.

  • You can shuffle the row order for respondents.

  • To limit respondents to one choice per column, at the bottom right of the question, click "More" (three dots).

  • To shuffle the row order, at the bottom right of the question, "More" (three dots).

Date

  • Respondents can fill out any date in this box.

  • To include the year or time, at the bottom right of the question, click "More" (three dots).

Time

  • Respondents can fill out the time or duration.

  • To switch between time or duration, at the bottom right of the question, click "More" (three dots).
