Purpose
SkyVine ITAD shall be accountable for the management of its own transportation and outsourced transportation in a manner protective of worker health and safety, public health, and the environment.
SkyVine ITAD has established, implemented and maintains the following transportation procedure which outlines the requirements for initial and on-going due diligence.
Initial Due Diligence
COMPANY OWNED VEHICLES
SkyVine ITAD shall retain maintenance and vehicle inspection records and permits and licenses as required by legal requirements.
SkyVine ITAD shall maintain adequate insurance coverage, consistent with the material and method of transportation, and maintain an acceptable vehicle and driver safety record during the previous three years.
OUTSOURCED TRANSPORTERS
FOR Parcel Transporters (USPS, FedEx, UPS)
SkyVine ITAD shall complete the Transporter Checklist with information provided on the transporter’s website.
FOR LTL (less than truckload) or FTL (full truckload) Transporters
SkyVine ITAD shall send a Transporter Letter and Transporter Checklist to all potential LTL & FTL transporters.
Transporters shall sign the Transporter Letter, and complete the Transporter Checklist prior to any transportation being conducted for SkyVine ITAD.
FOR Broker Transporters
SkyVine ITAD shall send a Transporter Letter and Transporter Checklist to all potential Broker transporters.
SkyVine ITAD shall request from the Brokers their written procedure for their qualifications.
SkyVine ITAD will audit the Broker's qualifications and approve them.
SkyVine ITAD will pull 5 random BOLS and request qualifications for the drivers on those BOLS.
SkyVine ITAD will audit the Brokers system annually.
SkyVine ITAD shall verify that its outsourced transporters have all the necessary regulatory authorizations, maintain adequate insurance coverage, consistent with the material and method of transportation, and maintain an acceptable vehicle and driver safety record during the previous three years.
SkyVine ITAD shall review the transporter’s SAFER Report at http://safer.fmcsa.dot.gov/companysnapshot.aspx.
Enter the Transporter’s DOT Number, MC/MX Number or Company Name in the search box.
Once the SAFER Web Company snapshot is found, SkyVine ITAD shall print out and save the SAFER Web Company snapshot for that transporter.
Click on the “SMS Results” link in the upper right corner.
Click on the “Complete SMS Profile” link in the upper left corner. SkyVine ITAD shall print out and save the Complete SMS Profile for that transporter
Ensure the Safety Rating is “Satisfactory”. If the Safety Rating is “Unsatisfactory”, the transporter cannot be used.
If the Safety Rating is “Not Rated” , review the Out of Service Rates. The Transporter’s Out of Service Rates should be less than or equal to the national average.
After completion of steps 3-6, SkyVine ITAD shall either approve or disapprove the outsourced transporter.
On-Going Diligence

SkyVine ITAD shall retain maintenance and vehicle inspection records and permits and licenses as required by legal requirements.
SkyVine ITAD shall maintain adequate insurance coverage for its consistency with the material and method of transportation and maintain an acceptable vehicle and driver safety record during the previous three years.
OUTSOURCED TRANSPORTERS
At least annually, SkyVine ITAD shall conduct due diligence on outsourced transporters, including;
Review and print out the SAFER Web Company snapshot and the Complete SMS Profile for all outsourced transporters.
Ensure the Safety Rating is “Satisfactory”. If the Safety Rating is “Unsatisfactory”, the transporter can no longer be used.
If permits, licenses or certificates of insurance have expired, SkyVine ITAD shall request and maintain updated records.
