Always exercise caution as you approach an accident scene. Inspect the scene before entering. Is it safe? If so, assess the status of the victim. Designate someone to notify the Operations Manager and Front Office Reception. Remain with the victim and provide comfort, reassurance and any emergency aid you are able to give until emergency medical technician (EMT) personnel arrive.
Give the Operations Manager/Front Office Reception the following information:
Location of incident.
Type of emergency: illness or injury
Gender of victim.
Approximate age of victim.
The Operations Manager/Front Office is responsible for calling (dial 8 for an outside line) 911 if the emergency merits it. If someone is not breathing or they have no pulse, and the Warehouse Manager or Front Office Reception is not available to call 911 immediately, YOU must call 911 immediately
The Operations Manager/Front Office Reception will assign an individual to watch for EMT personnel. The assigned individual will direct the EMT to the location of the victim.
If the medical emergency does not warrant immediate outside assistance, first aid kits are located in the office and Operations Manager’s office and directions to the nearest clinic/hospital are located at the front desk and in the Operations Manager’s office.
Once the emergency has been responded to, the Operations Manager or QEH&S Manager must complete an Incident Report. This report will be retained in the EH&S/HR office.
In the event you discover a small fire (waste-basket size), notify people in the area and send someone to notify the Operations Manager. If comfortable, use a fire extinguisher to extinguish the fire. Fire extinguishers are located at red support beams throughout the building as listed on the Facility map.
Identify if the fire puts you or others in imminent danger
If so, identify the nearest exit and follow the emergency response plan to notify warehouse personnel.
If you are not in imminent danger and time permits, wait for the warehouse manager to arrive and follow further instructions
If time does not permit:
first notify the surrounding employees in the area and instruct someone to notify a manager immediately
Utilize any tools listed below to contain and/or extinguish the fire. The best method being placing the battery in a suitable container, using a fire extinguisher to extinguish the fire, then covering the battery with a smoldering agent such as sand, kitty litter, or dense gravel.
Tools to Extinguish Fires
Fire Extinguishers identified by signage throughout the facility
Sand, gravel, and/or kitty litter to smolder fire located on the battery processing cart
Bucket to contain small battery fire to douse with smoldering agent such as sand or kitty litter
Tongs used to pick up small batteries to place into buckets.
Proper Extinguisher Usage
P: Pull the pin and break the seal at the top
A: Aim the nozzle of the extinguisher from a safe distance at the base of the fire
S: Squeeze the handles together to discharge the extinguishing agent
S: Sweep the nozzle side to side as you approach the fire directing the space at the base of the fire
Proper Extinguisher Type
Lithium metal batteries: Class D extinguisher (YELLOW in color)
Lithium Ion and all other batteries: Class ABC extinguisher (RED in color)
The Operations Manager will obtain the following information about the fire:
Location
Size
Are there any injuries?
If larger than a waste basket, the Operations Manager/Front Office Reception will call “911” to notify the Fire Department.
In the event of a fire the Operations Manager will contact everyone to evacuate the building.
Use the closest, but safest exits to evacuate the building. Facility maps are posted throughout the building. Note: Employees should know the two closest exit routes from their workstation.
Employees need to assist in ensuring everyone in their area is notified to evacuate, including contractors and visitors. EVERY EMPLOYEE, CONTRACTOR AND VISITOR MUST LEAVE THE BUILDING IMMEDIATELY UPON NOTICE TO EVACUATE.
The Operations Manager on site will attempt to inspect all areas of the building to ensure everyone has evacuated but will in no way endanger themselves.
Employees must report to the assigned safety zone located across Armstrong Street in the parking lot of 1015 Armstrong Street directly across the street from the SkyVine ITAD facility. In the event that the assigned safety zone becomes unsafe, the Operations Manager will direct employees to an alternate area.
After personnel are accounted for, an announcement will be made whether to allow reentry into the building or instruct personnel to go home and wait to be notified when the building will be reopened.
SPILL RESPONSE PROCEDURES
In the event you discover a spill, notify the people in the area and send someone to notify the Operations Manager or employee trained in spill response procedures.
Remember the 3 C's of handling any accidental spills. Control Contain and Clean up.
The Operations Manager or trained employee determine the specific substance of the spill by following the procedures listed below:
If the spill came from a SkyVine ITAD in-house material, go to the Safety Data Sheets for the proper clean up method.
If the spill came from a customer’s material, call the customer and ask what the material is and get the proper clean up method.
If the material is flammable (e.g., gasoline, or a solvent), make sure you extinguish all ignition sources immediately.
If the spill involves broken CRT glass, follow the Broken CRT Cleanup Procedure listed below.
If the spill involves broken fluorescent tubes, follow the Broken Fluorescent Tube Cleanup Procedure listed below.
If the spill involves oil, follow the Used Oil Cleanup Procedure listed below.
Based on the type of spill, clean up appropriately with the items provided in the spill kit bucket located in the Reusable Tech Room.
Wear the following personal protective equipment, provided in the spill kit:
Eye protection
gloves
If splash potential exists, supplement with protective apron & full-face shield
All spill and spill cleanup supplies are disposed of properly based on the type of spill.
After cleanup of the spill has been completed, notify the Operations Manager who will complete a spill incident report.
The Operations Manager will ensure the spill kit has been restocked after use.
BROKEN CRT CLEANUP PROCEDURE
CRTs in computer monitors and televisions contain lead and cadmium, which can be toxic to human health and the environment if not properly managed.
CRTs in computer monitors and televisions are handled and stored in a manner that prevents breakage, however when breakage occurs, it should be cleaned up and contained properly to reduce possibility of exposure and contamination.
Cleanup Procedure
Employees wear the following safety equipment:
Gloves made of leather, or equivalent.
Dust Mask
Safety glasses with side shields or full-face shields.
Safety toed shoes or boots.
Use a broom and dustpan to sweep the broken glass and materials. Deposit into a sealable bucket, drum or Gaylord container that will not allow glass leakage. (Never place broken or intact CRT’s in the garbage.)
The container is labeled “Universal Waste – CRT Glass for Recycling”.
The container is shipped to the approved vendor.
BROKEN FLUORESCENT TUBE CLEANUP PROCEDURE
Fluorescent tubes contain mercury which is toxic to human health and the environment if not properly managed. Mercury is the only heavy metal that is liquid at room temperature. It is more harmful to inhale the vapor from a bead of mercury than to ingest the same bead. At room temperature mercury vaporizes readily into an invisible, odorless, and tasteless poison.
Fluorescent tubes are handled and stored in a manner that prevents breakage, however if it occurs, it should be cleaned up and contained properly to reduce exposure and contamination.
Cleanup Procedure
Employees wear the following safety equipment:
Gloves made of leather, or equivalent.
Dust Mask
Safety glasses with side shields or full-face shields.
Safety toed shoes or boots.

LEAKING/SMOKING/FLAMING BATTERY PROCEDURES
Use care while handling batteries during testing, sorting, and disassembly activities. Puncturing, bending, or dropping batteries can cause damage to the battery that may result in a battery that is leaking, smoking, or that could combust resulting in fire. Extra care should be taken while handling a battery that appears damaged in any way or is swollen.
If a battery is leaking, place the battery in a sealed bag and place in the appropriate container in the universal waste area.
Wear the appropriate PPE as necessary when handling the leaking battery
Heat Resistant Gloves
Use tongs or scoop to handle the battery
Notify your supervisor and the Facilities Manager
In the event you discover a smoking or flaming battery, notify people in the area to evacuate the area and send someone to notify the Facilities Manager.
Protect yourself and avoid breathing the smoke.
If you feel comfortable extinguishing the battery - use the nearest fire extinguisher.
Remember the Acronym P.A.S.S when using the Fire Extinguisher
Pull – the pin
Aim – the nozzle
Squeeze – the trigger/handle
Sweep – from side to side at the base of the fire.
Once the reaction has been controlled take the remnants of the battery and place battery in the Battery Disposal container
Wear the appropriate PPE as necessary when handling the smoking or extinguished battery
Heat Resistant Gloves
Use the Tongs to handle the battery
After the fire has been extinguished, notify the Facility Manager who will clear the department to return to the area.
The Facility Manager will ensure the fire extinguishers are restocked/serviced after use.
Once the battery incident has been responded to, the Facility Manager must complete an Incident Report. This report will be retained according to the Records Management Table.
USED OIL CLEAN UP PROCEDURE
Oil spills are dangerous to the health of people and to the environment. The hazards include slips and falls, fumes, ignitions, asphyxiation, burns, water contamination, soil contamination and fire. Oil spills should be addressed immediately, contained properly and cleaned up to reduce exposure and contamination.
Cleanup Procedure
Employees wear the following safety equipment:
Gloves made of rubber, or equivalent.
Safety goggles or glasses with side shields, or full-face shield.
Safety toed shoes or boots
Stand by fire extinguishers
Control the spill. Attempt to stop the cause or source of oil or slow down the rate of release of oil.
Contain the oil spill by preventing the spill from spreading to other areas with the use of absorbent pads, absorbent rolls or containment socks. If the oil spill is near any type of drainage be sure to seal the drains first or protect with containment socks.
To cleanup, place the absorbent pads directly on to the oil spill.
Continue placing and replacing absorbent pads until the last drop of liquid is absorbed
Place all the soiled absorbent pads and socks inside a plastic bag. Re-bag soiled absorbents to prevent leakage.
Once you have finished with bagging everything, place the bags inside a drum or a sealed container (5-gallon bucket or 55-gallon drum).
The container is labeled “Used Oil Rags and Absorbent Materials”.
The container is shipped to the approved vendor.
If an employee is made aware of, or is concerned about the local weather condition, they should immediately contact the Operations Manager. The employee will inform the Front Office of:
Any Severe Weather Alerts they are aware of.
The source of the information. (i.e. National Weather Service, customer, truck driver, personal phone call)
If the National Weather Service issues a warning, or if the community’s sirens are heard, the Front Office will begin to monitor the weather using spotters and the weather on local radio and TV stations as needed. Operations Manager evaluates the situation and initiates a Severe Weather Alert as needed.
If severe weather develops in the area, the Operations Manager will notify office staff and the warehouse staff to proceed to the severe weather/tornado shelter room.
Employees/contractors/visitors are to proceed to the designated Tornado Shelter room immediately in an orderly manner. (Facility map posted throughout the facility shows the location of the Tornado Shelter room.) Stay away from all outside windows.
Once inside the Tornado Shelter room, the Operations Manager will ensure all personnel are present or accounted for.
The Operations Manager will give the “All Clear” once severe weather has left the vicinity.
POLICY
SkyVine ITAD considers the safety of its employees, or any individual on Company property to be of paramount importance. The Company has zero tolerance for any type of workplace violence committed by, or against, an employee, including threatening, intimidating, or inferring violence against any person or property associated with the Company.
Non‐Emergency (threatening) Response Procedure
One person, through intimidating words or gestures, has induced fear and apprehension of physical or other harm in another person but there is no immediate danger of such harm being inflicted.
Immediately notify your supervisor or another member of management.
Any suspicious individuals or activity must also be reported immediately.
Emergency Response Procedure
A situation is an emergency if an injury has occurred or there is an immediate threat of physical harm.
You should consider your personal safety first in all emergency situations.
Quickly assesses the situation and risk.
Call 911.
Immediately notify your supervisor or another member of management.
Investigations
SkyVine ITAD considers any threat of violence or potential violence as legitimate, and will take immediate appropriate action, including an investigation of the matter and involvement of law enforcement of any level.
Prohibited Conduct
The following conduct is prohibited at all times while on Company property or at any location while representing the Company:
Fighting or provoking a fight;
Horseplay;
Hostile behavior that creates a reasonable fear of harm to others or property; and
Intentionally damaging Company or coworker property.
Firearms and Weapons
An employee is prohibited from possessing firearms or weapons of any kind while on Company premises; in Company, rental or personal vehicles while conducting business for SkyVine ITAD or at customer locations, or any other location during working hours or while representing the Company, regardless of whether the employee is licensed to carry the weapon. The only exceptions are law enforcement personnel and security personnel.
Policy Violations
Violations of this policy will result in immediate disciplinary action, up to and including termination and/or legal action.
Recordkeeping
HR is responsible to complete an Incident Report for all workplace violence indents. This report will be retained according to the Records Management Table.
