Scope
To describe the process for identification of health and safety hazards and risks related to SkyVine ITAD activities, products, and services.
Responsibility and Authority
The QEHS Management Representative shall have the responsibility and authority to ensure conformance to the activities described in this procedure.
Procedure
The QEHS Management Representative, in conjunction with relevant employees, shall assess employee health and safety hazards according to the PPE Hazards Assessment Program. Hazards and required PPE shall be recorded on the PPE Hazard Assessment Form.
In addition, The QEHS Management Representative, in conjunction with relevant employees shall conduct on an ongoing basis (e.g., as new types of materials are processed, or new processes are utilized) a hazards identification and assessment of occupational and environmental risks that exist or could reasonably be expected to develop at the facility. Such risks could result from any sources, including but not limited to emissions of and/or exposure to substances, noise, ergonomic factors, thermal stress, substandard machine guarding, cuts and abrasions, etc. Hazard and risk assessment shall include:
Routine and non-routine activities, such as set-up, breakdown or an emergency,
Activities of all persons having access to the workplace (including contractors and visitors).
Human behaviors, capabilities and other human factors.
Identifying hazards originating outside the workplace capable of adversely affecting the health and safety of persons under the control of SkyVine ITAD within the workplace.
Hazards created in the vicinity of the workplace by work-related activities under control of the Company.
Infrastructure, equipment and materials at the workplace, whether provided by SkyVine ITAD or others.
Changes or proposed changes in SkyVine ITAD, its activities, or materials.
Modification to the QEHS management system, including temporary changes, and their impacts on operations, processes, and activities.
Any applicable legal operations relating to risk assessment and implementation of necessary controls.
The design of work areas, processes, installations, machinery/equipment, operating procedures and work, including their adaptation to human capabilities.
The Hazards identification and Risk assessment shall be documented on the OHS Risk Assessment.
Once hazards and risks are identified by process area, the QEHS Management Representative and top management shall make a determination of those hazards and risks that could have a significant impact on health and safety. This process includes a 1 to 5 ranking of the “likelihood” (1 is unlikely - less than once per year, 5 is probable - daily) times the “consequence” (1 is no effect, 5 is permanent) of the potential health and safety impact. The resulting “rating” determines the significance. Rankings with a 20 or higher are considered significant.
Health and Safety hazards shall be documented on the OHS Risk Assessment and defined further on the Significant EHS Aspects and Hazards Worksheet. Hazards that are required by OSHA to have a written program shall also be documented on the Significant EHS Aspects and Hazards Worksheet regardless of their rating.
Hazards identification and Risk assessment, and related objectives and targets, shall be reviewed for adequacy at least yearly during Management Review meetings.
Hazards identification and Risk assessment shall also be reviewed when,
Any major changes to activities and processes take place,
Any proposed changes to activities and processes are considered,
Any relevant laws or regulations are enacted or revised,
Any changes to the QEH&S Policy are made
