Patient medical histories
You need to submit histories for two different patients.
The information can be collected for a new patient or an already existing patient.
If obtaining a history for an already existing patient, the patient needs be presenting for a new medical issue. Submission cannot be a history on a patient presenting for a recheck or an existing lifelong problem.
Look at the criteria sheet for details on what information should be included in your patient history. Client information may be blacked out if necessary.
