This skill will require you to admit patients, discharge patients, take patient histories and maintain records. You will also learn how to prepare appropriate certificates and documents for signature. Upon completion of this skill, you will be able to effectively communicate with clients, discharge patients and prepare official documents as outlined in the skill description.
Tips for materials required for submission
You need to submit both a surgery/anesthesia form as well as a euthanasia form used at your site.
These can be blank, meaning they do not need to contain client / patient information.
A copy of a completed rabies certificate/ vaccine form.
This needs to be completed by you; so the date that the rabies vaccine was administered should be during your approved externship time frame.
Discharge instructions for five post-surgical patients who have had any surgical procedure performed.
Each surgical procedure needs to be different, for example, you cannot use multiple dental patients.
Do not submit pre-made or "canned" hospital discharge instructions given to the client at patient discharge. This needs to be a written description of the details you discussed with the client at discharge and why the point was important. The following points should be discussed but are not limited to:
Food/ water restrictions
Incision care
Activity restrictions
Bathing
Medications
Bandages
Potential post surgical complications
Example: You need to discuss how to check and care for the incision. Why it is important to take proper care of the incision? Why should the pet be prevented from licking it? What can happen if it is not properly cared for?
