Blank CEU form (This is how you will submit PD from a source other than District help PD)
This is the form you must complete and submit with accompanying artifacts (transcript, certificate of completion, etc.)
You will need
Your File Folder number
license renewal year
a digital copy of the CEU form for the session (1 of the 2 listed above)
for events OUTSIDE of the school district: digital artifacts, like a transcript or a certificate
