Vocational Assessments


There are three assessments that need to be completed for students turning 12 and up:

★ Level 1 Vocational Interview - Student

★ Level 1 Vocational Interview - Parent/Guardian

★ Level 1 Vocational - Teacher Assessment


The special education teacher must complete the vocational assessments for all students ​on their caseload.


How to complete the Vocational Assessments:


1. Complete the student interview - ask the student the questions and write their responses

2. Add information from the student interview to the child’s PLOP to include student voice

3. Complete the parent/guardian interview - ask the parent the questions and write their responses - this can be done over the phone, during a parent meeting, or during the IEP meeting

4. Add information from the parent interview to the PLOP to include parent voice

5. Complete the teacher assessment

6. Put “Level 1 Vocational Assessment - [month] [year]” in the “evaluation results” section of the PLOP

7. In SESIS, go to:

  • “Create New Document”
  • “Documents Related to Assessment”
  • Add comment: “Vocational Assessment”

8. Once the document has been created, go to:

  • “Print”
  • “Fax coversheet”
  • Print the fax coversheet

9. Fax into SESIS the 3 parts of the vocational assessment with the fax coversheet

10. For students turning 15 and up, use the information from the vocational assessment to complete the following two sections of the IEP (see “Transition Sections” document for guidance):

a. Measurable Postsecondary Goals

b. Coordinated Set of Transition Activities
