Who are the faculty members pursuing graduate degrees enjoying scholarship/financial grants either by the university or other funding institutions (present in tabular form indicating the following: name of faculty, degree pursued, granting institution, amount of financial grant, year started and year expected to finish/graduate;
upload all xerox copies of TOR & Diploma for BS and graduate degree of all faculty members teaching in the criminology program to include board rating/certificate and License;
Copy of the Special order or MOA for production engagement of faculty members if any;
Xerox/scanned copy of the accomplished evaluation tool used by the screening committee duly signed by the members and if possible to include copies of the medical and psychological exam;
Measures/ Policies to avoid in breeding;

Certified copy of the result of the screening of faculty applicants;
In tabular presentation, submit the summary of faculty workload (format: Name of faculty, course handled, corresponding number of students, lecture/laboratory, other designation, permanent/COS) duly certified by the program head and the campus registrar for the past 3 semesters to include the current semester;
submit the complete duly approved faculty workload for the past 3 semesters to include the current semester.
Proof that the faculty manual is published (send the website/link);
Copies of approved admin arrangement in cases of faculty leaves/absences;
Copy/ies of duly signed Contract of Service of faculty for the past 3 semesters to include the current semester;
Copy/ies of termination order, renewal, extension ( received copy from HR Office);
Copy (file copy from program head/Dean/HR Office) of the approved FDP of the college under survey (please highlight the criminology; instructors/professors);
Xerox/scanned copies of the certificate of active membership in professional & honor society among crim professors/instructors;
Copy of the approved budget allocation for faculty development;
Evidence showing the selection of deserving faculty to be granted scholarship, to attend in seminars, conferences and trainings (shoeing the criteria/basis);
Evidence showing the budget utilization for faculty development for the past 3 years.
