Project Open Hand's Wellness Program serves people who need nutritious food to support their health and/or medical treatment, helping them to get stronger and lead healthier lives.. Our evidence-based research proves that nutritious Food is Medicine.
For our neighbors fighting illness, good nutrition can help them heal quicker, get stronger faster, and lead healthier lives.
Medically Tailored Meals: We provide nutritious meals to clients living with critical illness. Meals are available for pickup once a week in our San Francisco and Oakland locations. For clients who are homebound, we offer home-delivered meals.
Fresh Groceries: For clients and caregivers who are able to prepare meals, we offer fresh, healthy groceries available for pickup once a week.
Mobile Groceries: For clients who are unable to travel to our locations, our mobile van provides fresh groceries and frozen meals at mobile locations throughout San Francisco and Oakland.
Nutrition Counseling and Education: To activate services with Project Open Hand, clients receive a nutrition and wellness assessment from our Registered Dietitians to guarantee our medically tailored meals fully support their dietary needs. Our dietitians offer regular check-ins and continued nutrition guidance, recipes, and recommendations for improved health outcomes.
Residents who do not meet requirements for eligible diagnoses may be eligible for Project Open Hand services through our CalAIM Community Supports Program or for our Community Nutrition Program.

Alameda County
HIV/AIDS
Recent major surgery, within 30 days of discharge (6-week service)
San Francisco County
Chronic Obstructive Pulmonary Disease (COPD)
Congestive Heart Failure (CHF)
Coronary Artery Disease (CAD)
Type 2 Diabetes, with an HbA1c of 8.0% or above
End Stage Renal Disease
HIV/AIDS

How to Apply:
Step 1: Confirm that you have an eligible diagnosis from the drop-down menu above. Note that your diagnosis must be one of the eligible diagnoses based on your county of residence.
Step 2: A licensed medical provider must fill out and sign the application below for the applicable county.
Note for providers: Eligible licensed medical providers included RN, NP, MD, DO, RD or RDN, LCSW or PA. The client's verbal consent on the application is OK.
Step 3: Either the client or the provider may send in the complete application to Project Open Hand via fax, email, mail, or in-person. Information on how to submit the completed application can be found on the application form.
Step 4: Project Open Hand's Client Services will evaluate the individual's eligible and, if eligible, will contact the client to set up services. If the client is not eligible for services, Project Open Hand will contact the referring party or provider to notify them.
